Your First Appointment

Complete a New Patient Registration Packet.  Please download, print, and complete these forms ahead of time and bring them with you to your first appointment.

If you are unable to complete these forms prior to your appointment, please arrive 20 minutes early to complete them.

If you are scheduled for a skin cancer check or laser treatment, do not wear make-up to your appointment or be prepared to remove your make-up.

Children under 18 must be accompanied by a parent or legal guardian.

Insurance Plans

We do not directly accept insurance plans other than medicare. However we will be happy to provide an itemized form for patients to submit to their insurance provider.

Payment Policy

• Payment must be paid at the time of service.
• We will provide an itemized form for patients to submit to their insurance provider. Be aware that not all insurance plans cover all services. Check with your provider for details.
